Your Journey to
Lasting Stability.
Healthcare doesn’t end when you leave the clinic. Our Care Pathways provide the professional oversight and human connection you need to stay in control — every day, at home.
Detailed Care Pathways
Each pathway is condition-specific, clinically designed, and managed by trained teams. Select your condition to explore what your family member receives.
Take Control of Your
Glucose, Every Week
Designed for anyone living with Type 1 or Type 2 diabetes. Your Metabolic and Renal Health scores are tracked continuously against both clinical safe ranges and your own personal baseline. We watch for polyuria, peripheral neuropathy, blurred vision, and slow wound healing at every check-in — patterns that, taken together, often show up before an HbA1c result confirms them.
Who It’s For
Newly diagnosed members, those with uncontrolled glucose levels, recently discharged members, and caregivers managing diabetes for a parent or family member abroad.
✔
Weekly Glucose Monitoring
Regular check-ins to track trends and catch fluctuations early

✔
A structured monitoring programme for members with high blood pressure. Your Hypertension and Cardiovascular Health scores are tracked continuously against both clinical safe ranges and your own personal baseline. A symptom checklist — headache, visual change, chest tightness — is reviewed at every Care Partner interaction, built to catch a change before it becomes a crisis.
Who It’s For
Patients with diagnosed hypertension, those with borderline BP readings, individuals with a family history of stroke or heart disease, and post-cardiac members in home recovery.

Monthly Stability Certificate
How your monitoring intensity is set: your Care Partner and Medical Director assign it based on your Health Profile and current condition — not the plan you sign up for. Everyday Preventive means monthly check-ins for lower-risk members. Chronic Care Support means biweekly monitoring for an active diagnosis. Intensive Management means daily oversight, typically assigned after a hospital discharge through our Discharge Bridge programme. It adjusts automatically as your needs change, at no extra cost.
Standard Membership is billed in NGN. Diaspora memberships are billed monthly in USD or GBP. Contact us to confirm the right plan →

Common Questions
from Families
Don’t see your question? Our team responds within 24 hours on WhatsApp — no question is too small.
Is this the same as telemedicine?
No. Telemedicine is a one-off video consultation. Rikenda is continuous care — a fixed team that checks in regularly, tracks trends over time, and proactively manages your family member’s health between doctor visits. We complement the clinic; we don’t replace it.
How do I know my family member is actually being seen?
Who exactly is on the care team?
Your family member is assigned a dedicated Chronic Care Specialist (primary monitoring and ongoing follow-up), a nutritionist (dietary guidance), and a counsellor (behavioural support and long-term adherence). A Medical Director oversees all clinical decisions and countersigns every Monthly Stability Certificate.
Do I still need to see a doctor?
Yes — and Rikenda is designed to make those doctor visits far more productive. We handle everything between appointments and bring 30 days of structured data to each consultation. We escalate to the clinic whenever a concern warrants it.
How do I pay from abroad?
We accept payment in USD, GBP, and CAD via card or bank transfer. Monthly billing, no contracts, cancel anytime. We specifically designed the payment process for diaspora families.
Where is Rikenda currently available?
We are now enrolling founding members in Port Harcourt, Nigeria, with plans to expand across Nigeria as the model is validated. If your family member is elsewhere, join the waitlist and we will notify you when we reach your location.
