Continuous Care Is Not a Luxury.
It Is a Public Health Obligation.
Rikenda is working to make ongoing support and follow-up a more consistent part of chronic disease care in Nigeria. Through community engagement, healthcare partnerships and practical care coordination, we aim to help people stay informed about their health and follow through on agreed next steps.
Our approach is built around the realities of care delivery: people need more than a diagnosis or a single check-up. They also need support in managing what happens between appointments.

We started at a community screening in Rivers State, where we met patients under 29 with blood pressures above 200 mmHg — people in hypertensive crisis who did not know it, had no follow-up, and no one coming back for them. This page is about what we are doing so that does not happen again.
Five themes emerged consistently across our community survey. These findings inform every element of the Rikenda programme design.
